Coordinated Care – The Patient at the Center of Attention

Coordinated healthcare is entirely patient-focused. Treatment is tailored to individual needs. A key element is the comprehensive nature of the services provided. This means that the entire patient care process, from diagnosis and treatment to rehabilitation and monitoring, takes place within a set timeframe and in one location. Throughout this time, the patient is cared for by a multidisciplinary team of specialists.

 

The course and implementation of the patient's diagnostic and therapeutic process is supervised by a medical coordinator, who acts as a link between the physician providing assistance and the patient in need of care.
